• trying to conceive for 1 year or more
• up-to-date with GYN exams and PAP smear
• female partner age 42 or younger

To schedule your free consultation, please fill out and mail (or fax) our registration form (form 3) and medical history form (form 2). Previous infertility treatment records will also be helpful. Please call our main office at 410-296-6400 to confirm that we received them and notify the receptionist of the website only free consultation.
